How can a Social Prescribing Link Worker help me?

We can link you up with services in the community that can provide you with:

  • Support to make positive changes in your life
  • Someone to talk to about how you are feeling
  • Help with housing, benefits and financial problems
  • Advice on jobs, training, and education
  • Exercise, and sports
  • Arts, music, outdoors and creative activities and classes

How might my health improve?

  • More motivation and energy

  • More confidence

  • Fewer symptoms

If you think the service may help you or someone you care for, or to find out more please contact your Social Prescribing Link Worker by telephone or e-mail or ask your GP Practice Receptionist. We also accept referrals from your GP, other members of your Practice team, community and district nursing teams and hospital discharge teams.

NHS Digital Weight Management Programme

This programme offers free, online access to weight management services for those who have a BMI above 30 and who also have a diagnosis of diabetes or high blood pressure, or both. The BMI threshold is lower at 27.5 for those from Black, Asian and minority ethnic backgrounds.

The NHS Digital Weight Management Programme is an accessible and convenient way to help you develop healthier eating habits, be more active and lose weight. You will only be able to access this programme if you have a smartphone or computer with internet access.
